Home health care provides skilled nursing, rehabilitative, and home health aide services on an intermittent basis to homebound patients with a variety of medical conditions. These services are delivered under the order of and are directed by the individual’s physician. Services are available for non-homebound individuals through other programs provided by our agency.
Services provided by skilled nursing:
- Assessment and evaluation
- Education on diet, medications, and disease processes
- Wound care management (including wound vac)
- Catheter care
- Infusion therapy through peripheral and central lines
- Injections (including flu vaccine)
- Diabetic or other qualifying foot care
- Pain management
Johnson County also provides skilled nursing services for residents with chronic conditions who need follow-up care but do not meet the home health criteria.
Residents requiring homemaker services are referred to the Division of Senior Services.
Office consultations are available for diabetes, blood pressure, and other needs as referred by a physician.
Other services available in conjunction with home health care:
- Physical therapy
- Speech therapy
- Occupational therapy
- Medical social worker
- Home health aide
Who qualifies for home health care?
Any Johnson County resident who:
- Is homebound due to debilitating illness or disease,
- Requires a skilled service that is medically necessary or needs to be performed by a nurse or therapist,
- And is under physician-directed care.

- Medicare
- Medicaid
- Private insurance
- Self-pay
Hours of operation:
- 24 hours a day; 7 days a week
