Johnson County Community Health Services We would like five minutes of your time to answer these questions!
When Johnson County Community Health Services (JCCHS) and the Community Health Assessment Resource Team (CHART) conducted the 2007 Johnson County Community Assessment we identified three top local disease risks:
- Chronic Obstructive Pulmonary Disease
- Diabetes
- Heart Disease
In order to address those concerns we are focusing on these interventions:
- Elimination of tobacco use
- Increase in exercise and activity
- Making healthy food choices
Now we are trying to determine what efforts and resources in the county are already working toward these goals. By gathering information on existing resources we hope to identify gaps. By knowing where gaps exist, we will know where to target future wellness programs and policies in the county.
We realize that not all facilities are the same, nor do they have the same organizational structure. For the purpose of this survey we use the term “site” to identify your business, facility, organization, school or other entity.
